If you are a new patient of ours, you’ll have to fill out a Registration Form and an Intake Form for the service(s) you are visiting us for.
You can fill it out online and have it automatically submitted to us, or download it, fill out your information and then submit it yourself either by email or in person when you come for your appointment.
